And it may appear when console could not detect your ps4 firmware the firmware will be wrong, the file and folder will be naming is wrong or maybe the USB is the format is wrong
Please check that the USB storage device (USB stick, external hard drive) you are using to update the PS4 is formatted to FAT32 or exFAT.
Format USB to Fat32
- Insert USB flash to your computer
- right click in the assigned drive od USB flash drive
- then select the format
- just change the file system type to FAT322
